Patient referral is easy:
Simply log onto our Web-User page where the on-line referral forms can be filled in; registration and use of this on-line service is completely free.
Alternatively, please download the referral form (PDF) for the Cavendish Imaging Centre you wish to use (London referral form , Birmingham referral form , Oxford referral form) and return it to us by email, fax or post.
We also accept referral letters on headed paper that contain the following information: referrer contact details, patient's name and date of birth, the area of interest and, most importantly, the type and purpose of examination and whether you require a Radiology Report, or will make your own Reporting arrangements.
